PATIENT SERVICE PLAN AGREEMENT
We always endeavour to provide the highest quality of healthcare, in return we ask that patients treat staff and other service users with respect, notify us if they are unable to attend appointments and make payments promptly. Please see below for further details.
WHAT TO EXPECT FROM YOUR CONSULTANCY APPOINTMENT
During your initial consultation your therapist / consultant will ask you some questions about your symptoms and will then ask to assess the problem area. In order to properly assess you, your therapist may need to see the affected area. For a lower limb condition it would be useful if you could bring a pair of shorts to change into, similarly for an upper limb condition bringing a vest top to change into would be useful. For a lower back condition it would be useful to bring both shorts and a vest top so that the therapist can see the whole of your spine and lower limb, or problems contributing to your symptoms may not be picked up. After your assessment, your therapist will discuss their findings with you and propose a treatment plan tailored specifically to your own goals.
CONSENT TO TREATMENT
Medical and therapeutic problems, procedures and treatments can be at times confusing for patients. Our therapists will not begin any form of objective assessment, or indeed treatment unless you have given your consent. This may be given verbally, or for some interventions such as injections, written consent will be required. We make every effort to help you to fully understand our diagnosis, the recommended procedures and any treatment that may be involved. We welcome any queries on anything that you may be uncertain of, or if you simply require more information. We also require that you make us aware of any factors that could affect your treatment or diagnosis. If necessary, we can also send correspondence to your GP.
CANCELLATION POLICY
All treatments must be pre booked. Cancellation of an appointment must be made not less than 24 hours before the appointment time. If cancellation is made within 24 hours of the appointment, you will be liable for 100% of the appointment cost. In the unlikely event of us cancelling your appointment, due to staff sickness for example, an alternative appointment will be arranged. The client shall not transfer treatments to any other person. Treatment fees may be increased by Remedy Ltd at any time. Practitioners are subject to change without notice. The patient is liable for ALL unpaid invoices. This is particularly relevant to those who wish for their health insurance company to pay treatment invoices. Often there is an excess to be paid directly to Remedy Ltd.
LIABILITY & INTELLECTUAL PROPERTY
The information available on or through this Site, and the Services supplied via or in connection with this site or at the Remedy Ltd do not constitute medical advice and it is your responsibility to determine, through obtaining appropriate medical advice, that you are fit and well and that such contents and services are suitable for you. It is not our responsibility to do so. Before commencing any exercise regime, you should consult your doctor. It is also vital that you supply us with correct information about yourself. We cannot be liable for any incorrect information supplied by you to us. We try to make sure that all information contained on this website (and provided by us to you as part of any services) is correct, but, subject to the paragraph below, we do not accept any liability for any error or omission and exclude all liability for any action you (your legal representatives, heirs) may take or loss or injury you may suffer (direct or indirect including loss of pay, profit, opportunity or time, pain and suffering, any indirect, consequential or special loss, however arising) as a result of relying on any information on this website or provided through any service supplied by us to you. You, your legal representatives and your heirs release waive, discharge and covenant, not to sue Remedy Ltd and its instructors for any injury or death caused by their negligence or other acts.
PAYMENTS
When you are required to pay the full cost of your treatment, we will take payment by bank transfer. We will invoice you via email so you have all the details necessary should you need it for your insurer.
LATE PAYMENT
Any on-going treatment may be suspended or cancelled if our fees and expense are not paid in full when due. We can charge interest if you pay late. If you do not make any payment to us by the due date, we may charge interest to you on the overdue amount at the rate of 4% a year above the base lending rate of the Bank of England from time to time. Interest shall accrue on a daily basis from the due date until the date of actual payment of the overdue amount, whether before or after judgment. You must pay us interest together with any overdue amount. In the unfortunate event that we need to enforce payment from you, you will be liable for all the incurred costs.
MODIFICATION OF THESE TERMS OF USE
REMEDY Ltd reserves the right to change the terms, conditions, and notices under which the REMEDY is offered, including but not limited to the charges associated with the use of REMEDY.
